HACC is a program that is jointly funded by the Commonwealth Department of Health & Aging and Queensland Health. It is designed to help frail aged & elderly people and people with disabilities to live independently in their own homes, to participate in their communities and is delivered through Cherbourg Day Respite Centre.
It helps them to help themselves with tasks such as personal care, home & yard maintainance and getting around the community. HACC can also help carers with information, training & education. discussion in person for eligibility is the initial assessment for suitability.
Queensland Council of Carers (QCC) supports carers through the provision of free carer support kits, information and support, training, advocacy, counselling, carer support groups & representing carer issues to the local community.
