Personal Helpers and Mentors Service (PHaMs) is for people with mental health illness who experience difficulty living independently in the community because of the impact of their illness. The service does this by:

 

  • Supporting people to manage their daily activities
  • Helping to access the services they need as required such as accommodation, social support, health, welfare and employment services
  • Building personal capacity and self-reliance
  • Increasing levels of community participation
  • Providing increased opportunities for recovery for people whose lives are affected by mental health illness
  • Taking a strength based recovery approach.

 

Who is eligible?

A person does not need to have a formal clinical diagnosis of mental illness to be able to access the service. Participants must:

  • Be 16 years of age or over
  • Have a mental illness and have functional incapacity due to the mental illness
  • Be willing to participate in the service voluntarily

Through our partnership with Cherbourg Aboriginal Shire Council, SkillCentred is committed to working collaboratively with Council, service agencies and the Cherbourg community.

SkillCentred manages and operates the Cherbourg Community Café & Training Centre situated on Barambah Avenue, Cherbourg. Our friendly team open the café 5 days per week and offer a catering service for community events and agency meetings as required.

The centre provides an opportunity for members of Cherbourg (and the surrounding district) to access introductory accredited training in hospitality and retail through combined theory and hands-on skills development approach and by providing participants with an opportunity to undertake ‘real’ work in the café.

  • Family and child connect provides referral pathways that enable families to access the right supports at the right time, building on their own strengths.

Safe Haven provides additional support to families, young people and the community to reduce the impact of domestic violence on children in the community.

 

What does Safe Haven do?

 

Family support workers – provide support and assistance to families who witness or experience domestic and family violence to develop skills and knowledge to care for their children, increase their capacity to manage and resolve complex issues and provide information regarding parenting issues.

 

Youth Worker – provides support to young people who experience or witness domestic and family violence. This service works towards enabling youth people to address the social/emotional issues that confront them in their daily life as they make the transition from adolescence to adulthood as a contributing member of society.

 

Community patrol – provides a prevention and intervention service addressing the safety and wellbeing of young people found wandering the streets of Cherbourg at night. The Safe Haven Patrol operates Wednesday through to Saturday between 4:30pm to 1:00am.

 

Community Capacity – works in collaboration and partnership with other networks and organisations to build and improve their capacity to provide prevention and intervention activities to children, young people and families who witness or experience domestic and family violence.

RENEW Intensive Family Support (IFS) works with families under stress through immediate assistance; practical hands on assistance; linking families to other specialist help and advocating with the family to get the help they need. This will allow families to work on a single case plan which includes all the support they need to help families be safe, resilient and to thrive.

 

Active engagement utilising people and services with whom the family is comfortable; meeting people at a safe place of their choice; rapid take up of referral; focussing on immediate assistance through practical help and utilising a strength based, solution focused model with families.

 

Practical, concentrated hands on assistance. Initial support will be intensive and will include:

  • Developing a single case plan
  • Providing supported referrals to specialists including clinical or therapeutic services
  • Linking the family to supports such as housing, emergency relief, child care, tutoring
  • Providing practical in home support such as developing daily routines, embedding behaviour strategies for children, budgeting, shopping and cooking

Ongoing engagement with the family to ensure the strategies from the single case plan are embedded and continue. Families will also be encouraged to be involved in groups around support or involvement of their children.

 

 

Who is eligible?

IFS will support families where:

  • There is a child or young person (unborn to 18 years) and
  • The family has multiple and/or complex needs and
  • The family would benefit from access to intensive and specialist support services and
  • Without support the child, young person and family are at risk of entering or re-entering the statutory child protection system and

The child is not currently in need of protection
